One of my patients recently experienced firsthand how fractured our healthcare system can be.

She went to the ER with worsening cancer symptoms. Because no beds were available, she was transferred to another hospital system. There, without knowing her full history or her existing oncology plan, physicians performed an MRI and recommended urgent brain surgery.

My reaction? “Whoooa, wait a minute! Is this real? Can a patient’s entire treatment trajectory change in the blink of an eye without consulting her original team?” Apparently so—if a patient agrees. But here’s the troubling reality: this patient has a brain tumor. How could she be expected to make such a critical decision, under pressure, without her established care team’s guidance?

That’s when I stepped into action.

She was confused, overwhelmed, and left questioning what had just happened. Her trusted oncology team had been treating her with a carefully coordinated plan of care, yet the new system discharged her when she declined their recommended surgery.

As her advocate, I immediately intervened. I spoke with the case manager and nurses, reminded them she already had an established oncology team, and secured her MRI images. I hand-delivered those results back to her original doctors so they could provide the guidance she truly needed.

And here’s the outcome that matters most: after being discharged home, she returned to her trusted oncology team at her original facility. Her physicians carefully reviewed the new MRI and discussed her case in full. They agreed that the other hospital should have consulted them before recommending urgent surgery. 

Ultimately, her established team confirmed a coordinated plan of care that did not include an emergency operation. Instead, they chose a more conservative, evidence-based approach—continuing her chemotherapy, immunotherapy, and radiation oncology, while also planning a less invasive procedure for her brain lesion.

This collaborative decision, made with full context and input from her established providers, ensures her treatment path is consistent, thoughtful, and aligned with her overall health goals.
