A 15-minute phone consultation to determine if a patient advocate can assist you with your medical needs.
We will complete an assessment, working with you to develop the scope of work and services. Prior to the assessment, we will send a contract and payment link to cover the cost.
Following the assessment, we will send an Advocacy Care plan which includes the scope of work to assist you with navigating your care. We will follow up with a phone call to review the assessment and care plan and answer any questions. At that time, the plan is yours to keep or engage with us to help manage your care.
We will get started on your customized plan once we have set up and received a retainer.
